An exciting opportunity has arisen to work within our well-established Adult Community Team, which delivers services to achieve the best outcomes for adults and older people who have care and support needs.

We work with a variety of people over the age of 18 who have the need for social work intervention. We are looking for a highly motivated, registered social worker to join our expanding Adult Community Team. We are proud and committed to working alongside people and their carers, to enable them to lead more independent and fulfilling lives, as part of their local communities.
 
We are specifically looking for a social worker to be part of our Older Persons Community Mental Health Team.
 
It is essential that you have a social work qualification and have a clear understanding of the Care Act and the Local Authorities statutory duties.

The successful applicant will also need to be a dynamic, innovative and committed person, willing to consider all possible options, working with external stakeholders, people and their families, as well as colleagues within local authority. They will have experience of working with older people experiencing mental health difficulties.
 
It is an exciting time to join us as we embark on a new model of social work practice which will be more relationship based and utilise traditional social work skills.
